Step 1: Client Information

TIP: To get a quote, you must be logged in. Visit I'm New Here for steps to login.

  1. Hover over Quotes & Enroll in the navigation bar under the VSP® Vision Care logo.
  1. Click on Create Quote. You’ll be directed to a new page Create a Quote.

  1. Enter the client's legal name.
  2. Select the state where the client is headquartered.
  3. Enter the month and year the vision plan will be effective.
      If you can't find the effective date you're looking for, the rates are not yet available.
  4. Choose the appropriate Population Type.
  5. Select the Client Status. A Prospective Client doesn’t have an active policy with VSP Vision Care. An Existing Client has an active VSP policy.
  6. Select a range for the number of employees expected to enroll.
  7. Choose the VSP Signature Plan® or VSP Choice Plan® for your client.
  8. Then, click Next: Customize Plan.

Step 2: Customize Plan

  1. Under Product Offering, select who will pay the premium. If you choose Packaged, another field will appear. Select the benefit(s) that the vision plan will be packaged with. If you choose Voluntary, additional fields will appear. Select your client's percentage of contribution and Expected Enrollment.
  1. Select the plan frequency. All plans include a vision exam every 12 months. Your client can choose if they want coverage for a frame and/or lenses every 12 or 24 months.
      You’ll see plan frequencies that are available to your client, based on the information you provided and selections made above.

TIP: Choosing a plan with coverage for lenses and frames every 12 months keeps out-of-pocket costs low for employees.

  1. Select a Rate Structure from the drop-down menu. The rate structure selected determines the monthly premiums you’ll see on the
    quote.
    4-tier – Premiums for Employee Only AND Employee + One AND Employee + Children AND Employee + Family
    3-tier – Premiums for Employee Only AND Employee + One AND Employee + Family
    2-tier – Premiums for Employee Only AND Employee + Family
    Composite – Single premium based on average risk of the group
    Employee Only – Premium for only the employee
  1. Once you select a Rate Structure, fields will appear to Select a Co-Payment. After a co-payment is selected, you’ll see a number of options to customize your client’s vision plan.
  2. Select the allowance for frames and contact lenses.

TIP: Did you know the average price for a frame today is $230. Choose a frame allowance of $180 or more to keep out-of-pocket costs low.

  1. Choose lens enhancements and additional benefits that best suit your client and their employees’ vision and eye health needs.
      You’ll see lens enhancements and additional benefits that are available to your client, based on the information you provided and selections made above.
  2. Then, click Add Quote. Your customized vision plan and rates will appear on the right of your screen.
  1. If you want to create another vision plan for the same client, repeat the steps above. Otherwise, click Next: Review.
      You can create up to four plans at a time for a client.

Step 3: Review Plan

  1. Review your client information and plan customization. If everything looks right, click Create Proposal & Save Quote. Alternatively, If you've only generated a single quote for one vision plan, you will see another button: Save Quote & Enroll. Click this button if your client has already selected the plan, and you're ready to  start a new application.
  2. If you want to make changes, click Back.
